Privacy Notice - Information for Patients
For accurate identification of your records we require you to enter details of your
- name
- date of birth
- phone number
We also offer an option to enter your email address.
This page explains how we handle that information, and the implications of either entering or not entering your email address.
We respect and protect your confidentiality and privacy. This system does not store any information about you, it merely forwards by email to the practice the appointment or prescription or other details that you have selected and the identifying details about yourself that you enter.
If you enter your email address then:
- The system will also send to you an email confirming the details of your appointment booking or prescription or other details.
- If you use a shared computer or email address and you do not want others to know that you have made a booking or prescription request etc then do not enter your email address.
- Email to the practice confirming your booking/prescription carries your email address as the sender. If this fails to be delivered to the practice for any reason then you may receive the "bounce" notice from the mailserver and you will be alerted to telephone the practice to confirm the online request.

In the case of an online appointment booking the slot which you reserved will still be available for you to confirm by telephone up until the evening before (Nobody else will have booked it since it will have been removed from the online system as soon as you booked it).
- If you receive an appointment "bounce" message please contact us. This will lessen confusion, otherwise you may turn up at the practice and find that we are not expecting you.
- In the case of prescription orders which "bounce" you may either re-order online or telephone a fresh prescription request.
- Note that this arrangement is simply a failsafe in case of lost emails - in normal operation we shall receive an email confirming your appointment booking or prescription.
You should also be aware that if you use a web browser that has an "autocomplete" facility then this may reveal to another person that you have used the system, since if they enter the first letters of your name then the browser may prompt them with your full name. If this is a concern for you then please do not use this system but make a telephone booking instead.
When using this online system please be aware of the points above.
- If you do not accept them at all then do not use the system.
- If you are happy with them apart from email confirmation then do not enter your email address.
- If you prefer
- to receive an email confirmation
- to have the failsafe of having the practice email returned to you in case of delivery failure
- and you are not concerned by the points above
- then please enter your email address when requested.
